Police failings contributed to domestic violence killings

An inquest has found that police failings contributed to the death of Rachael Slack and her two-year-old son. The two were killed by Rachael's ex-partner on 2 June 2010. Rajeev Thacker was instructed for the Slack family.

The jury found that despite the fact that Rachael and Auden were considered to be at high-risk of homicide, Rachael was not told this and, therefore, could not take the necessary steps to address the danger that she and her son were in.

In response to domestic violence killings like Rachael's, Refuge are calling for a public inquiry to help identify why so many women and children are not getting the protection they need from state agencies, including the police. The Slack family support this public inquiry.
